To begin enrolment please bring:
- W2 or Last Year’s Tax Return
- Birth Certificate or Legal Resident Card
- Proof of U.S Citizenship + Residency Status
- SSN + Date of Birth for Each Household Family Member Enrolling
- Proof of Property Assets (Such as Current Bank Statement)
