• Health Screenings
• Get Enrolled in Health Insurance

 

To begin enrolment please bring:

• W2 or Last Year’s Tax Return
• Birth Certificate or Legal Resident Card
• Proof of U.S Citizenship + Residency Status
• SSN + Date of Birth for Each Household Family Member Enrolling
• Proof of Property Assets (Such as Current Bank Statement)

Additional Event Offerings:

• Childcare Provided  On-Site
• Raffle for Thanksgiving Turkeys + Other Prizes
